How to sign up for the Foreclosure Prevention Workshop
1. Complete the following forms
- Intake Form (PDF, 57 KB)
- Monthly Budget (PDF, 30 KB)

Official intake form (with NeighborWorks® HomeOwnership Center Sacramento Region logo) must be provided.
2. Make copies of ALL the following documents
- Last federal income tax return
- First two pages ONLY
- Last two months of bank statements
- Most recent one months pay stubs
- Most recent mortgage statement from your lender
- Must state balance and monthly payment amount
- Mortgage Note (sometimes called Promissory Note, or Adjustable Rate Note)
- Located in original loan documents
- Must state original amount borrowed and interest rate
- SSA-SSI-Pension Awards Letters (if applicable)
- Profit and Loss Statement, (if self-employed)
3. Submit completed packet (steps 1 and 2)
Incomplete packets are NOT accepted

4. Wait to hear from us
- We process documents every Friday.
- Assuming your document packet was complete; you can expect to receive a call between 3:30-5PM the Friday after you submitted your documents.
- Incomplete packets are returned to the address provided.
5. Attend a workshop and follow through with counseling.
Appointment Policy
Our time slots are limited and we would like to be able to accommodate other customers if you cannot make your appointment.
Please call (916) 452-5356 ext 210 at least 24 hours before the time of the appointment if you need to cancel or reschedule the appointment.
We reserve the right to close your file
- if you reschedule more than four (4) times
- if you do not show up to two (2) appointments
- if you cancel your appointment without rescheduling.
